This memo provides implementation instructions and information for a newly developed Suspicion Based Drug Testing (SBDT) electronic ordering process through Comprehensive Drug Testing (CDT) at https://comprehensivedrugtesting.com/.

To streamline the SBDT referral process, clients who have met an SBDT indicator case and are being sent for SBDT testing will be referred using an electronic ordering process. Effective July 1, 2025, SBDT referrals will be handled via the CDT electronic ordering system. The following forms will no longer be used:

1. ES-4108 Collection Site Passport
   The ES-4108 Collection Site Passport will be replaced by the Donor Pass which is created and printed through the electronic ordering process. Only in the case that electronic ordering is not available can the ES-4108 Collection Site Passport be utilized.
   
   
2. Chain of Command/Custody Form
   The Chain of Command/Custody form will be replaced by the Donor Pass which is created and printed through the electronic referral process. Only in cases where electronic ordering is not available, and the ES-4108 Collection Site Passport is utilized should the Chain of Command/Custody form be used.
